Please bring the following with you at the time of your initial consultation:
- Insurance card
- Photo ID
- Physical Therapy prescription from referring physician
- Exercise shorts, athletic shoes
Payment in full is expected at the time of your visit. Except: Medicare, and worker's compensation. Cancellation of scheduled appointment required 24 hours advance notice or subject to cancellation fee.
